All of this worked exactly as expected until fairly recently (maybe the iOS upgrade to 13.x). For safety, I also checked "My Photo Stream" in the "Photos Settings" which should save 1 month worth of photos (especially before I can download them). This always meant that my camera roll would be empty, but every photo would exist exactly twice - on my phone and on my Mac, both in "Photos". Whenever I take pictures with my iPhone, I download them to "Photos" selecting "Delete after Import", then go to iTunes to sync my Mac with my phone (settings: "Sync Photos" checked, Copy photos from Photos, "All photos and albums" checked.) (BTW, I run 10.14.6 on my Mac).
